The relationship hits a rocky point and you break up, because at the time you don't see any other option. If you later, say within a week, realize that you could have handled it differently and fixed the relationship, is it too late to try to get back together and make amends? Its never too late to realize that one is wrong and ask for forgiveness, we all human and mistakes we all do. Try to talk with your ex explain your feelings and accept your wrongs and best of all ask for forgiveness. If you were to a point that you even said hurtful words to your ex apologize for that. If your ex decides to accept your offer to reconcile and work out things, then good congratulations beforehand but if not well don't worry at least you tried and failed than failed to try.

That depends on the person you are with. The best answer to this question is to go with your instincts because you know this person better than any one who could possibly answer your question.
If you were wrong and you care about the person, then be mature and admit your fault ;-)
You need to tell your ex you were wrong, and would like to have another chance. You need to do this before they find someone else.
